Have you tried the New Vegas stutter remover? Also enabling Vsync in the Control Panel/ENB settings and disabling the Vsync in the Fallout.ini?

New Vegas stutter remover has an ini file. Open it with note pad and change "bHookLightCriticalSections" from 1 to 0 It worked for me and my friends.

@"NeoSnow" ok so SWEET it looks like that did it however I think that save game was busted so I went back to an earlier save and that seemed to do the trick
