In terms of quests, I really liked Couriers Cache, the quest itself is well-done and you get some badass armor at the end.

I really like the Deathsong Brotherhood I thought it was a pretty good quest mod except its not completely finished and stops right in the middle of the story line.

Can't forget the Cube Experimental for Fallout 3, got a major Portal vibe from it which was cool

The Inheritance is a good one. After you've completed the main quest line, you get some quests that follow up your "success"

New Vegas Killer was hilarious.
